Most organizations plan for and continually strive to prevent incidents, including those related to occupational and process safety. Fortunately, process safety incidents are generally low-frequency; as such, internal staff typically do not have much exposure to conducting these types of incident investigations. In addition, it can be difficult for organizations to find “independent” personnel to conduct an unbiased investigation of the event.
Effective incident investigations provide the opportunity to learn about and improve safety performance. Kestrel works with organizations to lead and/or support larger and more complex occupational and process safety incident investigations. This can range from being the independent leader of an investigation to facilitating and supporting internal leaders, to providing input and expertise to internal investigation teams using company processes, to using our Human Performance Reliability (HPR) approach.
